Donald Trump walked up to reporters at the White House on Thursday to deliver an update on Iran. “The war is going very well, as you know. We’re winning by a lot. Iran is making very big concessions. We’ll see what happens, but it’s been very, very powerful. It’s going very, very well,” he said.

The problem was that between every few words, the president of the United States appeared to be struggling to catch his breath.

The footage aired live on Fox News and hit X shortly afterwards.

“Trump appears to have difficulty breathing and speaking in this clip,” one user posted alongside the video.

The reactions quickly followed.

There was more context behind Thursday’s footage than just one press gaggle.

The Haberman and Swan book Regime Change, out this week, added another dimension to Thursday’s footage. “He was also having trouble hearing, asking people to repeat questions they had just asked,” Haberman and Swan wrote. “Joint press conferences with world leaders were more often held in the Oval Office than in the East Room, in part because the acoustics were better, and he didn’t have to stand for an hour.”

The book also noted that “those who spent time with him could see the signs — the moments of fatigue, the cupped hand behind the ear,” and that “whatever thin verbal filter he had in the past was gone.”

Trump’s niece Mary Trump, a psychologist, added her own assessment this week. “He may still have moments when he appears more coherent, but psychologically he’s in a downward spiral,” she wrote, adding: “He’s experiencing constant narcissistic injuries, and nothing terrifies Donald more than humiliation.” White House spokesman Steven Cheung responded by calling her “a stone-cold loser who doesn’t have a clue about anything.”
